CARE FACILITATOR

 Classification:                    Youth Care

 Reports to:                           Coordinator of Care Management and Clinical Services

 Minimum Qualifications: 

 ·        Possess a recognized child-care worker diploma or a Bachelor’s degree in the human services field

·        Registration with the appropriate professional association

·        Minimum of five (5) years’ direct child care experience

·        Minimum of two (2) years adolescent residential experience

·        Ongoing development of knowledge and skills

·        Training on an annual basis to meet or exceed the standards of their professional association

·        Meet or exceed the minimum requirements of the Province of Nova Scotia, Department of Community Services, Provincial Standards for Residential Child-Caring Facilities; Children and Family Services Act and Regulations;

 Definition of Terms:

 Care Management is the term used to describe the coordination for the care planning process which includes placement planning, family history and dynamics, behavioural and emotional concerns, academic needs, life skills and leisure planning, and physical health.

 Clinical is defined as precise analysis of systemic and interpersonal issues in the lives of youth and families and utilization of this analysis into each element of the care management process, including consultation, collaboration and implementation.

 POSITION SUMMARY

 This position is responsible for ensuring the Care Management System is followed for each resident who is categorized as a Placement Breakdown.  While carrying out his/her duties, provide leadership to all staff in the delivery of the programs to ensure the highest levels of care and services to the Clientele of ADCRF facilities.

 DUTIES AND RESPONSIBILITIES

 In all duties and responsibilities and any delegation of tasks, the incumbent is responsible to ensure that all matters under the Provincial Standards for Residential Child-Caring Facilities which fall under their responsibilities are routinely reviewed and that the Standards are met or exceeded.

 ·        Regular attendance at the 9:15 a.m. shift change

·        Daily reading of the resident logs

·        Gathering all relevant background information commencing with the intake and compiling and communicating in a user friendly format to the Team as soon as it is available

·        Meet with the youth’s family and provide information regarding program and expectations

·        Ongoing involvement of family, formal closure meeting with the family at time of transition/discharge

·        Ongoing and relationship building with the agency workers, providing information regarding program and expectations

·        Communication with the CRPC regarding long term placement

·        Follow up on after hours call for emergencies (to agency worker and family)

·        In coordination with the youth care team, approving overnight passes within the context of the care plan

·        In coordination with the youth care team, arranging appointments and transportation

·        Arranging case conferences

·        Liaison and relationship building with therapists and community supports

·        Assessment of academic/vocational and recreation/leisure educational needs in consultation with other appropriate staff

·        Preparation of weekly of case updates and the placement of updates in the Communications Book and individual logs

·        Coordination of transition to next placement, including incorporating placement facility staff into transition where appropriate

·        Closure meeting with resident, family, appropriate support persons (youth support worker, staff, etc.)

·        Preparation and delivery of the Resident Transition Report within the guidelines of the ADCRF policies and the Provincial Standards

·        Knowledge and utilization of progressive youth care approaches

·        Commitment to the delivery of youth care through multi-disciplinary expertise

·        Other duties as may be assigned

 WORKING CONDITIONS

 The work environment demands strong concentration and the ability to focus while maintaining a sensitivity and appreciation of the delicate balance required with the clientele.  Work hours are based upon client needs.  Must be able to work independently and under stressful conditions.  Working environment often requires sensitivity to cultural, societal differences and appropriate protocols.  Working conditions include physical, emotional and mental demands.
